By clicking the above Online Registration Form, please find our new patient forms. For your convenience, you can complete and submit these forms securely online. E‑mail to us simply by clicking "submit" upon completion. Upon the first visit to The Perio & Implant Center, we do request identity and your signature on these forms at the time of check in.
We are located in two areas to serve you:
- In Monterey in the Ryan Ranch Commercial Park, directly across the street from The Herald newspaper offices just past UPS, in the middle building #21.
- In Palo Alto, next door to Lucille Packard Children's Hospital and Stanford Medical Center, located at 777 Welch Road.

It is our goal to exceed your expectations in dental care, and in this we would love to know what we can do for your comfort during the short time in waiting while at the Perio & Implant Center. We have fresh herbal and fruit infused teas, Kona coffee, Italian Cappuccino, lattes and espresso, both hot and iced.
Our hygienist is highly skilled and experienced in Periodontal Therapies and will provide amenities like a heated neck or shoulder wrap, aromatherapy, eyewear and the ability to view the pictorial series "Planet Earth", for your comfort.
